If you purchased it in store, the cash register printed rebate form will have the following zip code for the address:

88565-1485. This is WRONG.

The correct one should be as shown in the attached MIR pdf form in OP's post.

Yep that has been my experience as well. Even though technically Young America now owns GFS they still operate as an independent company. The last two D-Link rebates I did were SLOW. They took 5 months and that was only after two phone calls to get them going.

I was shocked that I went into my store about 3 hours after opening, and there were 6 of these sitting on the shelf. Looks like OD finally realized that if something is on sale, they should have more than 1 in stock

I ordered one around 11am this morning for instore pickup. Figured it would be nice to have, but if I didn't get it I could live without it. I stopped at the store around 3:30 on my way to the gym and they actually had pulled it and had it ready for pickup. I was surprised - this is one of the first times I've actually had a smooth in-store pickup.

I bought this at a store at Manchester CT. The rebate form printed at cashier shows:

......
PO Box 751485
EL PASO
TX 88565-1485

I believe the zip should be 88575 since the PO is 751485. Online form shows 88575. Be careful.
